Insurance & Payment
Provide your insurance information when applicable. Our office can discuss insurance and available payment options with you. Coverage and benefits vary by individual plan. Affordable self-pay options are also available.
Complete Your New Patient Forms
Complete the required new-patient intake information before your appointment whenever possible. This provides your healthcare provider with important medical, psychiatric, medication, and treatment information.
Your First Appointment
Your first appointment is an opportunity for us to understand what brought you to Greenway Recovery & Wellness. Your provider will review your history, current concerns, medications, treatment goals, and other information relevant to your care. Please bring a current photo ID, insurance information when applicable, and an accurate list of your current medications.
Your Individual Treatment Plan
Following your evaluation, your provider will discuss appropriate treatment recommendations with you. Treatment is individualized. Medication, including controlled medication, is prescribed only when clinically appropriate following evaluation and is not guaranteed simply because an appointment has been scheduled.
Moving Forward
If Greenway Recovery & Wellness is an appropriate setting for your care, follow-up appointments will be scheduled according to your individual treatment needs. Our goal is not simply to get you through your first appointment. Our goal is to help you keep moving forward.
What to Bring to Your First Appointment
- Current photo ID
- Insurance card (if applicable)
- Accurate list of current medications, including dosages
- Completed new patient intake forms (if provided in advance)
- Any relevant medical or psychiatric records you wish to share
